SUPPLEMENTAL CONDITIONS <br /> • FOR ENCROACHMENT PERMIT NO. 66481 <br /> The following are included as part of the conditions for approval of Encroachment Permit no 66481 <br /> 1 Keep one lane open to traffic in both directions at all times Lane closures are <br /> only permitted between 9 am and 3 pm <br /> 2 All lane closures shall conform to the Caltrans Traffic Control Manual <br /> 3 The boring backfill material shall be a two (2) sack sand and cement slurry <br /> capped with eight (8) inches of a six (6) sack cement mix, dyed with enough lamp <br /> black to appear homogenous with the surrounding asphalt <br /> Failure to comply with the above conditions shall cause this permit to be immediately <br /> revoked Revocation notice may be by verbal notification by an authorized <br /> representative of the Public Works Director or by written correspondence <br /> eGEOIRErM NTROSS <br /> • JUNIOR ENGINEER <br />
