Lyon: <br /> Pacific Environmental Group, Inc. (PACIFIC)has scheduled the installation of <br /> three groundwater monitoring wells (U-1, U-2, and U-3) at the above referenced <br /> site for October 23, 1990. The San Joaquin County Department of Public Health <br /> Services issued a letter to UNOCAL on September 27, 1990, requesting additional <br /> items prior to the issuance of permits for these wells. The following information is <br /> provided in response to this letter: <br /> o A completed permit application and remittance of permit <br /> application fees (check number 5255 in the amount of$89.00 are <br /> enclosed). <br /> o The drilling contractor for this activity will be Western Hazmat <br /> Drilling (California Licensed Contractor C57-554979). <br /> o A Site Health and Safety Plan is enclosed. <br /> o A site map showing underground utilities is enclosed. There are <br /> no leach fields on site,but two septic tanks are present as <br /> indicated on the enclosed map. <br /> One undocumented water supply well has been identified on site, and its position <br /> is also shown on the site map. Water from this well is used for sanitation purposes <br /> only.
