The field map from the <br /> Site Approval package, which apparently was not forwarded to you,has been sent <br /> to you, and another copy is included with this letter. <br /> • As to Appendix B, the dates of the water samples are correct. <br /> • Regarding Water Sample Locations, see the Surface Water Samples Location <br /> Map and the Well Water Samples Location Map in Appendix B,just after the <br /> FEMA FIRM Flood zone map, and just prior to the Laboratory Analysis Results <br /> & Chain of Custody. These maps are also listed on the blue sheet at the <br /> beginning of Appendix B. <br /> • The addresses of the Domestic Wells which were over legal maximum for Nitrate <br /> are as follows: <br /> DW-4: Ed Nunes Dairy LPC, 17391 E.
