Please provide PHS/EHD <br /> certification in writing that all record fee title owners have been notified of your request <br /> for closure. Submit a list of all the record fee title owners with their mailing addresses to <br /> PHS/EHD so they can be notified of our intent to issue a closure letter. If you are the <br /> sole property owner, provide a statement of that fact to PHS/EHD, to fulfil this <br /> requirement. <br /> In addition, the monitoring wells at the site must now be destroyed. Please submit to <br /> PHS/EHD a permit application, $60 permit fee and verification of destruction method. <br /> Monitoring wells MW101, MW1 and MW2 must be overdrilled and grouted.
