1 have completed the MFR form to the best of my ability. It appears to have little or no relevance to the work <br /> we are performing at 7500 W 11th. Street in Tracy. This is the first time 1 have encountered this type of form. <br /> We have worked at the site for over 13 years, but have never been called on for such, or similar, documents <br /> before. I have no idea which Board of Supervisors District the site is in or what Key1 and Key2 are, or what <br /> relevance they may have to this project. <br /> The permitting fee that Mr. Dietz paid when he visited your office was what he was told to pay. Assuming that <br /> the amount was in error we will mail the additional amount ASAP. <br /> With respect to the San Joaquin County Public Works Department encroachment permits, of which there are <br /> two, those were supplied to your staff at the time we made application for the well permits. An additional check submitted for the amount of$1,100.00. [The check that you submitted was for <br /> $3775.00. The cost for destroying each well is$125.00. Also there is an inspectionlactivity fee of$375.00 per <br /> parcel. The San Joaquin County Environmental Health Department considers all wells in the public right-of-way <br /> to be on one parcel, making the total count of 4 parcels.
