Missing p pp <br /> submitted on 03 May 2013 were the following: <br /> 1. Master File Record (MFR)form; (I have appended a copy of the form for you to fill out and submit.) <br /> 2. Encroachment permit issued by the San Joaquin County Public Works Department for the wells <br /> located in the public right-of-way; <br /> 3. An additional check submitted for the amount of$1,100.00. [The check that you submitted was for <br /> $3775.00. The cost for destroying each well is$125.00. Also there is an inspection/activity fee of <br />[ $375.00 per parcel. The San Joaquin County Environmental Health Department considers all wells <br /> in the public right-of-way to be on one parcel, making the total count of 4 parcels.
