The requirement for item 3 <br /> is amended to read as follows: <br /> "If the existing well is to be destroyed, a permit from the <br /> Environmental Health Division shall be acquired before water <br /> will be supplied, and destruction of the well shall be complete <br /> within 30 days of hook-up. If a well is to remain on the <br /> property, an approved reduced pressure backflow prevention <br /> device will be required. The applicant shall have the reduced <br /> pressure backflow prevention device tested by a licensed tester <br /> prior to final inspection and every year therafter at the <br /> owner's expense.
