4 <br /> f <br /> M <br /> a CONDITIONS OF APPROVAL <br /> UP-67-12 <br /> DENNIS KULIK, ET AL <br /> 1 . Planning Division : <br />' a. A minimum of 16 parking spaces ( including one handicapped <br /> space) shall be designated on the Improvement Plan. <br /> b. Other than signs located an the building, signage shall be <br /> limited to one double-faced free standing sign not to <br /> exceed 32 square feet per side and 35 - feet in height . The <br /> location of the sign shall be designated on the <br /> Improvement Plan. <br /> c. Hours of operation shall be limited to 6 : 00 a.m. to 10 : 00 <br /> p .m. <br /> d. Outdoor lighting shall be designed so that glare from the <br /> lighting shall not be visible from adjacent residential <br /> uses . <br /> 2 . De2artment of Public Works : <br /> a. An Improvement Plan prepared by a Registered Civil <br /> Engineer and showing drainage, driveway access details, <br /> and on-site parking shall be submitted to the Department <br /> of Public Works for approval prior to the issuance of a <br /> building permit . <br /> b . An encroachment permit shall be required for work within <br /> the road rights-of-way. <br /> 3 . Local Health District : <br /> a. Provide a recorded easement for the sewage disposal area <br /> and the required 100 percent expansion area. <br /> b .
