The <br /> RWD consists of the enclosed Form 204, a filing fee,and a technical report that will serve as your <br /> response to Sections V1 and Vll of Foran.200. The technical report must, at a minimum,address all <br /> items.listed on the enclosed sheet entitled"Additinnal Information Requirements"and must be <br /> prepared under the supervision of u California registered civil engineer.'The RWD shall also include <br /> any additional information required to explain the site-specific processes. <br /> The filing fee is based on the threat and complexity of the discharge,as described in the enclosed Filing <br /> Fee Schedule. Staff has determined that this discharge is classified as a 2B threadcomplexity,and <br /> therefore a check in the amount of$2,700 must accompany.the RWD. Because Schencne Slleci&1ty <br /> Foods,Inc. submitted a$400 check on 17 March 2003,the balance amount of$2,300 shat l be . <br /> submitted with the RWD.
