Employees have not had <br /> annual universal waste training. Employees who handle universal wastes (UW) must be trained initially and annually <br /> thereafter. At a minimum, personnel who manage UW shall trained and thoroughly familiar with proper universal waste <br /> management and emergency response procedures relative to those persons' responsibilities, including where <br /> applicable, <br /> 1. types and hazards associated with the UW managed at the facility <br /> 2. proper disposition of UW managed at the facility (e.g. locations of UW containers, or location of centralized UW <br /> area) <br /> 3. proper procedures for responding to releases of UW including the position titles and the means of contacting those <br /> personnel at the facility who are designated to respond to releases <br /> 4. labeling, collecting, handling, consolidating, and shipping UW at the facility, including, but not limited to the <br /> prohibition on the disposal of UW <br /> The UW handler shall maintain a written record by date indication the names of personnel who received the training and <br /> maintain those records for at least three years. Immediately provide employees with initial or refresher UW training. <br /> Submit a copy of the roster and the syllabus to the EHD. This is a Class II violation. <br /> 715 Failed to keep complete records of each shipment of universal waste for three years. Copies of universal waste <br /> disposal records for the last three years were not found on site. Universal waste disposal records shall be retained for <br /> three years and have them readily available for review. The record may take the form of a log, invoice, manifest, bill of <br /> lading, or other shipping document, and shall include the following: <br /> -Name&address of destination facility <br /> -Quantity &type of universal waste <br /> -Date of departure/shipment <br /> Immediately locate a copy of all universal waste disposal receipts for the last three years, maintain them on site, and <br /> submit copies to the EHD. This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by October 24, 2014. <br /> Consent to perform the inspection, take photos and collect samples was given by Jeremy Bongcaron, mechanic <br /> and owner's son. <br /> "REVISED REPORT""9/25/14 Added violation #712. <br /> Note: No containers were observed on site for the disposal of used antifreeze, spent absorbent, or waste aerosol <br /> cans. <br /> Note: This facility has not yet paid the annual EHD permit fees for 2014. These fees are past due and incurring <br /> penalties. <br /> This facility generated an unknown amount of hazardous waste in the last calendar year (Jan-Dec 2013).
