<br /> A report of waste discharge consists of a complete State Form 200 and a technical report that thoroughly
<br /> characterizes the discharge. The technical report must be signed and certified by a registered civil
<br /> engineer and must include the following information:
<br /> 1. Facility Description. Describe the facility in terms of what is accepted, how it is processed, and
<br /> what is produced. The description should quantify what is produced on a daily basis (e.g.,pounds of
<br /> fruit processed per day). Briefly describe significant plant equipment (e.g.,boilers, evaporators, ion
<br /> exchange units, reverse osmosis units) and its function within the process. Provide the hours of
<br /> operation and any seasonal variations in the operation of the facility.
<br /> 2. Waste Characteristics. Describe the quality of each component of the waste stream proposed to be
<br /> discharged to land (i.e.,process wastewater, wash water,boiler blowdown, etc). Describe any
<br /> pretreatment the wastewater receives prior to discharge and the removal efficiencies. The
<br /> description should include maximum and average flow and concentrations of BOD, suspended
<br /> matter, standard minerals', and total nitrogen of the discharge following pretreatment. Describe
<br /> daily or seasonal variations in the volume and/or quality of the discharge, how such parameters are
<br /> monitored, and how the flow rate of the discharge is measured.
<br /> 3. Process Flow Diagram. Provide a flow process diagram for the facility and its discharge. Indicate
<br /> sources and volumes of intake water, operations contributing wastewater to the effluent, and
<br /> treatment units. Construct a water balance indicating average daily and maximum daily flows (mgd)
<br /> between intakes, operations, treatment units, and outfalls.
<br /> 4. Chemical Usage. Provide a detailed accounting of the chemical usage at the facility for all
<br /> chemicals that enter the process stream (e.g., acids,bases, and salts) and a description of how and
<br /> where in the process stream they enter. Estimate the amount of chemical used on an annual basis
<br /> and describe any seasonal variability in chemical usage. Further, report any hazardous wastes that
<br /> maybe generated at the facility and certify that all hazardous wastes will be disposed of in
<br /> accordance with California Code of Regulation(CCR), Title 23, Chapter 15 and will not be
<br /> commingled with process wastewater.
<br /> 5. Vicinity Map. Provide a scale map based on the appropriate USGS 7 '/z Quadrangle map (or its
<br /> equivalent) showing the location and acreage of the wastewater application area. The map must
<br /> indicate all topographical features, the direction of groundwater flow, all domestic and irrigation
<br /> wells within 500 feet of the site, the direction of prevailing wind, all residences within 1!2-mile of
<br /> the site, and land use in the vicinity of the site.
<br /> 6. Site Map. Provide a scale map of the site showing the location and dimensions of all major
<br /> buildings,roads and parking areas,process and wastewater treatment structures, drainage control
<br /> structures, on-site wells, ponds, and wastewater application areas.
<br /> Standard Minerals include total dissolved solids,electrical conductivity,chloride,sulfate,nitrate,bicarbonate alkalinity,
<br /> carbonate alkalinity,calcium,magnesium,potassium, sodium,pH,hardness, silica,boron,iron,manganese,ammonia,and
<br /> phosphate.
