The attached two-part <br /> -questionnaire is designed to supplement the interview requirement of the SSSCR. <br /> V <br /> Part I includes questions which relate specifically to the user's responsibilities,and to additional information <br /> sources which will be useful to us in completing our investigation most efficiently. Please complete this <br /> portion of the questionnaire at your earliest convenience and return it to us promptly. <br /> Part 2 includes questions to be asked of the current property owner and/or occupants. Please forward a copy <br /> of this portion of the questionnaire to the current owner and request that they complete and return it as soon <br /> as possible.
