LIVE OAK 6EO ENVIRONMENTAL <br /> Surface A Subsurface Contamination Report - The Arbor Mobile Rome Park, Acampo <br /> June 30, 2009 <br /> incident of sewage overflow since PERC began monitoring the system in October <br /> (personal communication,June 30,2009). <br /> 6.0 SITE DOCUMENTS <br /> a, ti.T Environmental Health Department Records <br /> The EHD was contacted regarding the Site address of 19690 North Highway 99. LOGE <br /> reviewed records on file for the Site at the EHD on June 18,2009. The following incident <br /> reports were identified: <br /> • August 31, 1999 report of a drug lab discovered at unit #4. Staff of the San <br /> Joaquin County Sheriff's Department inspected the house and determined that <br /> there were no signs of contamination or disposal to the sewage system or <br /> ground. <br /> • June 21, 2008 report of surfacing sewage near unit #72. Surfacing sewage was <br /> reported as a "periodic problem." <br /> The EHD file also included copies of the Regional Board documentation for the Site. The <br /> Regional Board issued Waste Discharge Requirements (WDRs) for the wastewater <br /> system at the Site on June 12, 2003. Apparently the WDRs were issued in response to <br /> system malfunctions in 2002. According to the WDRs, the system utilizes a package <br /> treatment plant, 60 seepage pits, and a gravelless leachfield_ A Monitoring and <br /> Reporting Program (MRP) was also adopted for the Site by the Regional Board, <br /> including monitoring the influent prior to treatment by the package plant, monitoring <br /> the effluent prior to discharge to the pits or leachfield, visual inspection, and <br /> groundwater monitoring. Two Notices of Violation issued by the Regional Board were <br /> also on file. The first, from 2003, was for elevated levels of Total Suspended Solids and <br /> Biological Oxygen Demand in the effluent. The second, from 2004, was for wastewater <br /> pooled on the ground surface and failure to report the incident. Beyond a review of the <br /> file for citations, a determination as to whether the Site is in compliance with Regional <br /> Board requirements is beyond the scope of this report. <br /> LOGE searched the EHD well and septic permit files for the Site on June 9, 2009. The <br /> following permits were found: <br /> • 1983 permit to add six 1,600-gallon septic tanks to the existing system as <br /> polishing tanks and 12 seepage pits three feet in diameter by 25 feet deep. The <br /> permit depicts three preexisting tanks and 48 preexisting seepage pits. <br /> • 1988 well destruction permit. <br /> • 1988 well replacement permit. <br /> • 2000 permit for repair to well. <br /> • 2001 permit for addition of 4-foot by 48-foot leach line to existing septic system. <br /> • 2007 permit for addition of eight 60-inch wide by 25-foot deep seepage pits. <br /> i <br /> 6 <br /> w <br /> t <br />
