Weber Avenue, 3rd Floor �- <br /> Stockton, CA 95202-2708 0 <br /> ch <br /> Dear Dennis: <br /> Listed below are the items I have enclosed that were requirements necessary after your <br /> inspection here at The Record on 6/5/02. <br /> #3 Form B <br /> #4 Certification of Financial Responsibility and Letter from Chief Financial Officer. We will <br /> schedule this to be done annually. <br /> #18 Monitoring and Response Plan <br /> #20 We have started a Monitoring Log which is located in the Guard Shack in the back lot <br /> here at The Record, this will be completed on a daily basis. <br /> #23 Copy of 2002 Monitoring Certification. We missed scheduling an inspection in 2001 so no <br /> records are available; we will make sure and schedule this annually. <br /> #25 Triennial Tightness Test. On 7/2/02, I spoke to Mike Flores, Stockton Service Station <br /> Equipment Co., Inc. and he informed me that this test is not necessary on our system <br /> because we have a suction type system that is continuously monitored. I am not sure if he <br /> is correct, so could you please inform me if this still needs to be completed? If so, I will <br /> contact another company to do this test. <br /> I understand we have until 12/31/02 to perform the Secondary Containment test and 12/31/03 to <br /> install the under dispenser containment. Both of these will be completed at a later date. <br /> Please note I have also enclosed a copy of two separate letters addressed to your company <br /> informing them of a name change that took affect in 2001 but as of this date the name change <br /> has not been completed by your department.
