MAR-20-2007 10:04 FROM:STOCKTON TERMINAL 9483092 TO:3694228 P.3
aivaaaru• iv, a.vv. vv.v. rra re-wi: .
Surface and Subsurface Contamination Report Questionnaire
NOA Project Number: P07044
Property Location: 800 N. Shaw Road, Stockton, CA
Page 2 of 11
The Surface and Subsurface Contamination Report Questionnaire
The Surface and Subsurface Contamination Report (SSCR) satisfies San Joaquin County
requirements for parcel division and lot line adjustments and is intended to identify
any sources of contamination on the property from on- or off-site causes.
San Joaquin County requires that the current (and former, in some cases) owners of
the property be interviewed regarding, any environmental knowledge of the property
they may possess. The attached two-part questionnaire is designed to supplement
the interview requirement of the SSCR.
Part 1 includes questions which relate specifically to the user's responsibilities, and to
additional information sources which will be useful to us in completing our
investigation most efficiently. Please complete this portion of the questionnaire at
your earliest convenience and return it to us promptly.
Part 2 includes questions to be asked of the current property owner and/or
occupants. Please forward a copy of this portion of the questionnaire to the current
owner and request that they complete and return it as soon as possible. [If you
currently own the property and purchased it over six months ago, please complete
Part 2 as well.] If the property is currently developed for non-residential uses, a copy
of Part 2 should also be forwarded to each commercial tenant, with a request that it
be completed and returned.
We thank you for you assistance with this process.
/J)ns kurr anrgrvrsiiom rrhnru rlris yreslim»nrii.lleau comacv "l'ina Chene.r m(209)367-3701 err 233.
