<br /> tion and history of the existing or proposed plant, works, or system or
<br /> proposed changes therein,and showing the geographical location thereof
<br /> with relation to the source of the water supply and all the sanitary and
<br /> health conditions surrounding and affecting such supply and the plant,
<br /> works or system.
<br /> The plans,specifications,and statement shall be in such form and cover
<br /> such matters as the department prescribes.

<br /> 4013. Upon receipt of a petition filed pursuant to this chapter, the
<br /> department shall make a thorough investigation of the proposed or exist-
<br /> ing plant,works,system,or water supply,and all other circumstances and
<br /> conditions which it deems material.

<br /> 4014. As a part of the investigation, and after 10 days' notice by mail
<br /> to the petitioner, a hearing may be had before the department or an
<br /> examiner appointed by it. At the hearing all testimony shall be given
<br /> under oath, and evidence, oral and documentary, may be received, a
<br /> record of which shall be made and filed with the department. Notice of
<br /> any such hearing shall be given to the public by the petitioner in form
<br /> prescribed by the department.

<br /> 4015. If, upon the completion of the investigation, the department
<br /> determines that the water furnished or supplied, or proposed to be sup-
<br /> plied,is such that under all the circumstances and conditions it is not pure,
<br /> wholesome, and potable or may constitute a menace or danger to the
<br /> health or lives of human beings, or that the existing or proposed plant,
<br /> works,system or water supply,or proposed modifications,are unhealthful
<br /> or insanitary,or not suited to the production and delivery of pure,whole-
<br /> some,and potable water at all times for health and normal household use,
<br /> it shall deny the petition and order the petitioner to make such changes
<br /> as it deems necessary to secure a continuous and adequate supply of pure,
<br /> wholesome, healthful, and potable water.

<br /> 4016. If the department determines that the system will not be subject
<br /> to backsyphonage under normal operating conditions and that the water
<br /> to be furnished or supplied is such that under all the circumstances and
<br /> conditions it is pure, wholesome, and potable and will not endanger the
<br /> lives or health of human beings, it shall grant a permit authorizing the
<br /> petitioner to furnish or continue to furnish or supply water.

<br /> 4017. Any person who operates a public water system shall do all of the
<br /> following:
<br /> (a) Comply with primary and secondary drinking water standards.
<br /> (b) Ensure that the system will not be subject to backflow under nor-
<br /> mal operating conditions.
<br /> (c) Provide a reliable and adequate supply of pure,wholesome,health-
<br /> ful, and potable water.
