Hazelton Avenue <br /> Stockton, CA 95205 <br /> Dear Ms. Mueller: <br /> Caltrans has reviewed the above Application (PA-02-146) for a small winery in existing <br /> structures on the north side of State Route (SR) 12, 2158 feet west of Guard Road, Lodi <br /> (9301 W. SR 12, Lodi APN 025-080-02). Caltrans has the following comments: <br /> • Left turn channelization is not required at this time. However, if a safety and/or <br /> operational problem develops, channelization and/or other mitigation may be required. <br /> This would require an encroachment permit, and the following may apply: <br /> If an encroachment permit will be required from Caltrans the environmental document <br /> prepared for the project that includes Caltrans right of way must be submitted with the <br /> encroachment permit application. Caltrans also requires documentation that cultural <br /> (archaeological), biological, and hazardous waste surveys have been conducted within <br /> Caltrans right of way. <br /> For cultural surveys a recent record search from the information center and an <br /> Archaeological Survey Report are required. <br /> A Natural Environment Study report shall be written documenting the results of <br /> biological surveys and the record search from the California Department of Fish and <br /> Game Natural Diversity Database. A qualified biologist should conduct surveys at the <br /> appropriate time of year to determine if listed plant or animal species or wetlands occur in <br /> the area. Surveys should meet the protocol standards of the U.S. Fish and Wildlife <br /> Service and the California Department of Fish and Game. <br /> If right of way is being dedicated to Caltrans, the applicant is required to submit a copy of <br /> Attachment A, confirming that the land to be dedicated to Caltrans is free of hazardous <br /> waste.
