(1601 East Hazelton Avenue) Stockton,California 95201 6%q <br /> (209) 468-3400 <br /> TO: INSTALLERS AND OWNERS OF NEW UNDERGROUND STORAGE TANKS <br /> San Joaquin County Public Health Services Environmental Health Division has <br /> established the following guidelines to assist you in the completion of <br /> your construction project. The guidelines specify the construction and <br /> monitoring details needed to complete our review of your underground <br /> storage tank system. <br /> 1. SUBMITTAL OF PLANS <br /> a. Submit 4 complete sets of detailed drawings to the Environmental <br /> Health Division. One set will be retained by the department. <br /> b. Include equipment specification sheets and manufacturer' s <br /> brochures describing operating and monitoring systems. <br /> C. Include detailed monitoring and response plans. <br /> 2 . PLAN REVIEW AND OPERATING PERMIT FEES <br /> a. New facility plan review fee of $30. 00 and operating permit <br /> application fee of $150. 00 plus $50. 00 per additional tank is due <br /> at time of plan submittal plus $56. 00 per tank State surcharge. <br /> b. New installation of additional tank at existing facility is <br /> $30.00 plan review plus $50. 00 per new tank permit fee plus State <br /> surcharge. <br /> 3 . PLAN REVIEW GUIDELINES <br /> a. The enclosed plan review sheet is provided for your information <br /> to assist you in interpreting our requirements for new tank <br /> systems. <br /> b. A letter will be sent to you in the event deficiencies are found <br /> in the tank plans. <br /> C. A copy of the California Underground Storage Tank Regulations can <br /> be obtained from the State Water Resources Control Board <br /> (916) 324-1262 or from this office for a handling fee of $2 . 00. <br /> d. Please check with the San Joaquin County Air Pollution Control <br /> District, Building Department and Fire District for their <br /> requirements. <br /> e. Submittal of new tank plans to this agency satisfies tank <br /> registration requirements for the State of California. <br /> f. Please allow five (5) to fifteen (15) working days for plan <br /> review. <br /> g. Field inspections associated with the plan review process should <br /> be arranged twenty-four (24) hours in advance. <br /> h. A total of three (3) inspections will be made of the tank system <br /> being installed according to the enclosed inspection record. <br /> i. complete and submit an application to operate underground storage <br /> tank(s) . <br /> A Division of San Joaquin County Health Care Services <br />
