Other requirements may also apply. These <br /> requirements cannot be modified. <br /> A. A qualified environmental professional shall prepare a surface and subsurface <br /> contamination report, identifying any potential source of surface or subsurface <br /> contamination caused by past or current land uses. The report shall include evaluation <br /> of non-point sources, such as fuel storage tanks, septic systems, or chemical storage <br /> areas. The report shall be submitted to the Environmental Health Department at time of <br /> submittal of a tentative map (San Joaquin County Development Title, Section 9- <br /> 905.12). <br /> A review fee of $250 shall be paid at the time of submittal to the Environmental Health <br /> Department. <br /> NOTE: <br /> The Environmental Health Department has received a report dated July 10, 2013 and it <br /> has been approved. <br /> B. Submit an addendum to the previously approved soil suitability study (SR006342) <br /> dated September 2, 2011 , indicating that the area is suitable for septic system usage. <br /> The study must be approved by the Environmental Health Department prior to the <br /> recordation of parcel map. (San Joaquin County Development Title, Section 9-1105.2 <br /> (d)). A review fee of $250 must be paid at time of submittal to the Environmental Health <br /> Department. <br /> NOTE: <br /> 1 . Environmental Health Department has confirmed that a perc-test was performed on <br /> Parcel # 3 under permit number SR0062995 dated July 18, 2011 <br />
