Facility had not yet filled out an exclusion notification form, so I had them fill one <br /> out (used tire dealer, see attached). The facility had 199 waste tires onsite (an additional 139 used tires were stored <br /> properly for resale). The differences in waste tire counts on previous reports may be due to improper storage of used <br /> tires for resale. <br /> Facility was given an area of concern during the previous inspection for not submitting CalRecycle 204 form because <br /> facility stated that a CTL was not received from "Shamrock Recycling of California, Inc." DBA"Padilla Tire Disposal" <br /> (1284842)for a pickup of 120 waste tires on 9/9/2014(receipt attached). Hauler normally leaves a receipt with each <br /> CTL form. Facility only had a receipt for this pickup. <br /> In the TEA 22 Work Plan Instructions, regarding complaints it states in the Comments section write "Inspection in <br /> response to complaint/referral of(explanation). Inspection was pre-approved by(CalRecycle inspector liaison) on <br /> (date)." I see in the email chain Barbara did not want me to mention the complaint in the report. I just wanted to get <br /> clarification on whether or not I should do that. My contact is aware that it was a complaint inspection. <br /> Thanks, <br /> Harprit S.
