White Slough <br /> Water PollutionlOntrol Plant -6- 21 March 2000 <br /> Inspection Report <br /> 2. The Discharger should provide a fail-safe system to prevent excursions. One method could be <br /> staffing 24-hours per day. If the Discharger chooses to not staff the plant, 24-hours per day, <br /> then adequate measures must be taken to minimize the risk associated with excursions such as <br /> adequate alarms. The Discharger should install and operate a high and low alarm system for the <br /> chlorination and dechlorination units. An explanation on how the alarm and call-out system <br /> work should be provided. <br /> 3. Discharger should update and train operators of"warning signs" that a system unit process may <br /> be in distress. There is no reason that the swing shift operator-who made the last rounds of the <br /> chlorination system should not have recognized the anomalies on the dose and residual analyzer <br /> charts. These anomalies became discernable as early as 2100 hours on 3 January 2000. <br /> 4. In order to comply with Standard Provision and Reporting Requirement, General Provision A.6. <br /> The Discharger should update standard operating procedures (SOPs) for the facility. The <br /> when and how treatment processes will be operated and <br /> updated SOP needs to specify P P <br /> maintained. <br /> >. Pursuant to Standard Provision and Reporting Requirement, General Reporting Requirement <br /> B.3., the Discharger shall be required to submit to the Board a technical report on its preventive <br /> (failsafe) and contingency (cleanup) plans for controlling accidental discharges, and for <br /> minimizing the effects of such events.
