t ' <br /> Mr. Richard Prima -2- 21 March 2000 <br /> 2) Discharger shall submit current status and schedule for completion of alarm installations for <br /> chlorine analyzers. Include a thorough discussion of how the alarms will work and under what <br /> conditions the alarms are set to call out. <br /> 3) Discharger shall discuss plant-wide preventive measures that will be used to reduce the <br /> likelihood of future accidental releases. <br /> 4) Pursuant to Standard Provision and Reporting Requirement, General Reporting Requirement <br /> B.3., the Discharger shall present a contingency plan. The plan shall discuss what procedures <br /> will be used to minimize the effects of the discharge in the event of a plant upset or accidental <br /> discharge to surface waters. Additionally, a discussion of call out procedures is warranted (i.e. <br /> list of local and state agencies that will be notified in the event of accidental releases to surface <br /> waters). <br /> 5) Provide cost estimates for efforts necessary to prevent recurrence of the chlorine violation for: <br /> a) the proposed alarm system, including installation, maintenance and staffing costs; and, b) <br /> staffing of the treatment plant 24-hours per day, 7-days per week, so that staff are on duty to <br /> catch these problems. <br /> Please submit this report to our office by 24 April 2000. Failure to submit a complete report may <br /> result in an enforcement action or fine up to $1000 per day late. The Regional Board may also <br /> consider further enforcement action based on review of the submitted information.
