i
C
2. Tubing and Packer Specifications Injection willtake
place only through the 4 1/2 inch tubing with a Baker
Lok-Set Packer set within the long string casing at
approximately 892. feet above the injection zone. The
tubing -and packers shall be maintained to prevent the
-movement of fluids into an, underground source of drinking
water. -The tubing and packers shall be corrosion-
resistant and of' sufficient strength to resist wellbore
pressures developed during the life of the well. The
caning/tubing annulus will be filled with corrosion
inhibited fluid. Injection between-. the outermost casing
and the wellbore is prohibited.
3 . Injection Intervals -Injection into 'the intended -zone is
through perforations made through the steel casing and
cement into the zone of injection by 11-gram jet charges
at 4 shots per foot; 120 degrees phasing, in 3-1/8 - inch
liollow st'eel°" cArrier guns. Perforations are- 0. 31 inch
diameter holes penetrating 18.9 inches. Injection shall
`be permitted-for'--the lower;`Domengine Formation, between
4220 feet` and 4559 feet below"the ground- surface.
Alteration of the injection" perforations and other rework
operations must= be properly reported (EPA Form 7520-12)
and the well must demonstrate mechanical integrity before
injection is authorized.-
4 . Monitoring Devices—The- operator- shall install and
maintain in -good operating condition:
(a) A tap on the suction line prior to the injection
pump and the wellhead for the purpose of obtaining
representative samples of• the injection fluids;
(b) A pressure, temperature and flow transmitter to
measure and transmit to a- computer=based data
acquisition system the parameters lited in Part II,
Section D. 4 of this permit. The flow transmitter
and totalizer- shall yield measured cumulative
volumes that are certified for a deviation accuracy
of five (5) percent or less throughout the range of
injection rates allowed by the permit; and
" (c) A pressure -gauge designed to provide (1) a full
M ` '' pressure range of 100 percent greater than the
g anticipated operating pressure, and (2) a certified
deviation accuracy of five (5) percent or less.
5. Proposed Changes and Workovers The permittee shall give
advance notice to the Director, as soon as possible, of
Page 6 of 31
UIC Permit #CA194000002
