' Written approval by the <br /> Director is required for change of injection fluids. <br /> 7 . Reporting shall consist of <br /> average, maximum, and minimum daily-and monthly values <br /> for injection pressure, annular pressure, flow rate, <br /> 'temperature and volume. <br /> Temperature, annular pressure, and injection pressure <br /> shall be measured at the wellhead.
