i (B) Any noncompliance with a permit -condition, or malfunction of the injection system, which may cause fluid migration into. or between underground sources of drinking water. (ii) A written- submission shall also be provided within five (5) days of the time the permittee becomes aware of the..circumstances. The written submission- shall contain a description of.,the;noncompliance and its cause; the period-;of noncompliance; including exact dates and-times, and if, the .no_ ncompliance has not been corrected_, the anticipated time it is expected to continue; and steps taken or planned .to reduce, eliminate, and prevent r-ecurrence,of the noncompliance. Where the permittee becomes aware thatit failed to submit all relevant .facts in the permit application, or submitted incorrect information= in- a. ,permit •applicaton or -in any report to .the Director, the permittee shall submit such facts- or information, within two (2) weeks of the time such information becomes known. 11. Continuation of- Expiring Permits (a) Duty to Reapply. If the. permittee wishes to continue an activity regulated by this permit after the- expiration date ,of this _permit, the permittee must submit a._.comglete application for a new permit at least 180 days -before this permit expires.
