Duration of noncompliance, including exact dates and times of noncompliance, and if the:n.oncompliance continues, <br /> the time by which,compliance is reasonably expected.to.occur. <br /> c. All measures taken or to be taken to reduce, eliminate, and prevent recurrence of such an upset, slug, accidental. <br /> discharge,or other conditions of noncompliance. <br /> d.All accidental discharge reports shall be submitted to the City of Lodi, Public Works Director;221 W. Pine.Street, Lodi, <br /> CA 95240. <br /> PART 4-REOPENER CLAUSE <br /> A. This permit may be reopened and modified to incorporate any new or revised requirements resulting from the City of <br /> Lodi's reevaluation of its local limits. <br /> B. This permit may be.reopened and modified to incorporate any new or revised requirements developed by the City of <br /> Lodi as are necessary to.ensure POTW compliance with any new federal, state,or local regulations promulgated. <br /> PART 5-COMPLIANCE SCHEDULE <br /> i� Not Applicable. • <br /> U PART 6-SPECIAL CONDITIONS <br /> A. The City will supply well water to the facility for use in sinks,toilets, eyewashes and hosebibs with the agreement that <br /> at such time the City decides to replace the existing well that NCPA will share the cost of the replacement well including <br /> the cost of associated additional piping. All fixtures shall be posted with signs stating that the water is not designated for <br /> consumption. <br /> B.A minimum of 20%of all wastewater on a yearly basis must be injected with the deep well injection system. <br /> C. Operate and maintain a wastewater sampler, temperature recording, pH monitoring and flow monitoring equipment.. . <br /> Submit annual certification of calibration of flow and pH monitoring and temperature recording equipment. <br /> D.Submit monthly summaries of the following: <br /> 1. Flow to NCPA from secondary clarifiers. <br /> 2. Flow to WSWPCF. <br /> 3, pH of flow to WSWPCF. <br /> 4. Temperature of flow to WSWPCF, <br /> 5. Injection Well Summary. i <br /> E.Oillwater separator on storm drain system must be maintained to operate at maximum efficiency. <br /> F. Waste from cleaning of reverse osmosis membranes must not be discharged to the WSWPCF. <br /> 420NCPA <br />
