The 9 5/8" surface casing shall be cemented with sufficient cement to fill <br /> behind this casing from the shoo to the ground surface. <br /> 2. Mid fluid of sufficient weight and proper consistency to prevent blow-outs <br /> shall be used in drilling, and the column of mud fluid shall be maintained <br /> to the surface at all times, particularly while pulling the drill pipe. <br /> 3. Adequate blow-out prevention equipment shall be provided and maintained ready <br /> for operation at all times. <br /> 4. In the event the 5 1/2" casing is comented below the surface casing, fresh <br /> waters shall be protected by cement behind the casing. Sufficient cement <br /> shall be used to fill to at least 100' above the bass of fresh waters. <br /> S. This Division shalo be notified: <br /> a. To inspect blow-out prevention equipment prior to drilling below a depth <br /> of 10001. <br /> b.
