No. 077-20345 ,Section 14 • T. 3H ,R. 5E M.D. R. & M., <br /> --- field, --- area, --- pool, <br /> San Joaquin County, dated 10-12-79 ,received 10-25-79has been examined in conjunction with records <br /> filed in this office. <br /> THE PROPOSAL IS APPROVED PROVIDED THAT: <br /> 1. The 8 5/811 casing is cemented with sufficient cement to fill behind this casing <br /> from the shoe to the ground surface. <br /> 2. The 5 1/211 casing is cemented with sufficient cement to fill behind this casing <br /> from the shoe to at least 500 feet above the uppermost oil or gas zone. <br /> 3. Drilling fluid of a quality and in sufficient quantity to control all subsurface <br /> conditions in order to prevent blowouts is used. <br /> 4. Blowout prevention equipment conforming to Class III B 2M requirements is installed <br /> on the 8 5/811 casing and maintained in operating condition at all times. See Manual <br /> M07 and attached sheet. <br /> 5. Blowout prevention equipment conforming to Class "III" 3M requirements is installed <br /> on the 5 1/211 casing and maintained in operating condition at all times. See Manual <br /> M07 and attached sheet. <br /> 6. Water with any beneficial use is protected from contamination at all times. <br /> 7. THIS DIVISION IS NOTIFIED: <br /> a. Before deviating from the proposed casing program and/or placing any plugs in the <br /> hole. Additional requirements shall be outlined at that time. <br /> b. To witness a test of the installed blowout prevention equipment prior to drilling <br /> out the shoe of the 8 5/811 casing. The blind/blank rams shall be tested by the <br /> operator prior to calling the Division inspector. Test information of the blind/ <br /> blank ram test shall be entered on the tour sheet along with the signature of the <br /> person in charge. <br /> c. To witness a test of the 5 1/211 water shut-off. <br /> NOTE: 1. RECORDS FOR WORK DONE UNDER THIS APPROVAL ARE DUE WITHIN 60 DAYS AFTER COMPLET101 <br /> OF THE WORK AND/OR SUSPENSION OF OPERATIONS. <br /> 2.
