- jna <br /> Ks <br /> W All labels will be filled out using indelible ink and affixed to the sample containers via <br /> the adhesive backing, if appropriate. Wide, clear plastic tape will then be placed over <br /> the Iabei to protect it and to further secure the label'to the sample container. An <br /> example of the sample label to be used is given on Figure 4-2. <br /> 4.4.1.2 Chain-of-Custody Record <br /> A Chain-of-Custody Record will be filled out for each sample shipment container. All <br /> samvles within a container must be accounted for on the Chain-of-Custody Record. An <br /> example of the Chain-of-Custody Record is given on Figure 4-3. <br /> 4.4.13 Transfer of Custody and Shipment <br /> Samples are accompanied by a Chain-of-Custody Record. When transferring samples, <br /> the individuals relinquishing and receiving them sign, date, and note the time on the <br /> record. This record documents sample custody transfer from the sampler, often <br /> through another person, to the analyst at the laboratory. <br /> Samples are packages properly for shipment and dispatched to the appropriate labora- <br /> tory for analysis,with a separate Chain-of-Custody Record accompanying each shipping <br /> container. Shipping containers will be sealed with custody seals for shipment to the <br /> r ' laboratory. The method of shipment, courier name(s), and other pertinent information _ <br /> are entered in the Remarks Sect-ion of the Chain-of-Custody Record. An example of <br /> the custody seal is given on Figure 4-2. All shipments are accompanied by the Chain- <br /> of-Custody Record identifying its contents. <br /> If sent by mail, the package is register:d with return receipt requested. If sent by cam- <br /> mon carrier, a bill of lading is used. Freight bills, postal service receipts, and bills of <br /> lading are retained as part of the permanent documentation. <br /> Documents will be distributed as follows: <br /> Ship with samples: <br /> Chain-of-Custody Record (original) <br /> Return to CH2M HILL files: <br /> Chain-of-Custody Record (pink copy) <br /> - Shipping records <br /> 5I'031605�WPM4.5! 4-9 <br /> .a.,� � <br />
