<br /> 4.4.1.4 Laboratory Custody Procedures

<br /> F�tr A designated sample custodian accepts custody of the shipped samples and verifies that
<br /> the samples received match those on the Chain-of-Custody Records. Pertinent infc)r-
<br /> s mation as to shipment, pickup, and courier is entered in the Remarks section. The
<br /> custodian then enters the sample numbers into a logbook.
<br /> The laboratory custodian uses the sample identification label number or assigns a
<br /> unique laboratory number to each sample and is responsible for seeing that all samples
<br /> are transferred to the proper analyst or stored in the appropriate secure area.
<br /> Tile custodian distributes samples to tile. Appropriate analysts. Laboratory personnel
<br /> are responsible for the care and custody of samples from the time they are receivej
<br /> -ted or returned to the custodian. The date of sample analysis

<br /> When sample analyses and necessary CIA checks have been completed in thee labora-
<br /> tory, the Unused portion of the sample must be disposed of properly. All identifying
<br /> stickers, date sheets, and laboratory records are retained as part of the permanent
<br /> documentation. Sample containers and remaining sample material arc dispo..--d of
<br /> appropriately using standard laboratory procedures.
<br /> 4.4.1.5 Field Record Keeping
<br /> In addition to completion of Chain-of-Custody Record,soil boring logs,well completion
<br /> logs, and other preprinted data forms, a designated team member will be assigned a
<br /> bound field notebook in which sif!nific�nt events, observations and raeasarements tivi!}
<br /> r be recorded. This notebook is to be kept as a permanent record. Notebook copies
<br /> will be maintained in CH2M HILL Emeryville office files.
<br /> 4.4.1.6 Corrections to Documentation
<br /> Unless prohibited by weather conditions, all original data recorded in field notebooks,
<br /> sample identification tags, and Chain-of-Custody Records are written in waterproof ink.
<br /> If an error is made on an accountable decume:nt assigned to one individual, that indi-
<br /> vidwil may make_ corrections siml)ly by cl-Ossing a line through the error and entering
<br /> the correct information. The errotteot nforination should not be obliterated. Any
<br /> subsequent error discovered on an accountable document should be corrected by the
<br /> person who niade the entry. All subsequent corrections must be initialed and dated.
