Kupka, Owner ENVIR NPR a l �o <br /> Lodi Airport PER MENTAL HEALTH <br /> 23987 North Highway 99 MIT/SERVICES <br /> Acampo, CA 95220 <br /> Dear Mr. Kupka: <br /> INFORMATION REQUEST FOR LODI AIRPORT - 23987 NORTH HIGHWAY 99, <br /> ACAMPO, CALIFORNIA 95220 <br /> The California Department of Health Services (Department) has <br /> received information that the above mentioned facility owned or <br /> operated by your company has stored and/or handled hazardous <br /> materials as defined in the Attachment. <br /> Specifically, analysis results of samples taken by the Department <br /> revealed soil contamination with DDT, DDE, 2 , 4-D and toxaphene. <br /> As part of the investigation, the Department is requesting <br /> historical and present information from you as a present <br /> owner/operator of the above stated facility. Under Section <br /> 25358 . 1 of the California Health and Safety Code (H&SC) , any duly <br /> designated officer, employee, or representative of the State may <br /> require persons subject to Chapter 6. 8 of H&SC to furnish <br /> information necessary for the Department to determine the <br /> appropriate response to a release of hazardous substances or <br /> wastes. <br /> Therefore, the Department is requesting the information described <br /> in the Attachment to this letter pursuant to Chapter 6. 8 of the <br /> California Health and Safety Code. <br /> Compliance with the site specific request for information set <br /> forth in the Attachment is mandatory. Failure to respond fully <br /> and truthfully to this request within forty five (45) days of <br /> receipt of this letter, or to adequately justify such failure to <br /> respond, may result in enforcement action (enforcement action may <br /> include, but is not limited by civil penalties including fines of <br /> up to $25, 000 per day per violation) by the Department, pursuant <br /> to Chapter 6. 8 of the California Health and Safety Code (Sections <br /> 25353 . 1 and 25367) . <br />
