Fremont St. <br /> Stockton, CA 95205 <br /> San Joaquin County Public Health Services, Environmental Health (PHS-EHD)is pleased to notify you that the <br /> Central Valley Regional Water Quality Control Board has concurred with our decision that all required <br /> investigative and remedial activities have been adequately completed for the above noted site. <br /> In order to provide you with a"No Further Action"letter, the monitoring and vapor wells and vapor probes <br /> currently located on and off-site will need to be addressed. You have two options for the wells and probes. <br /> You may destroy them under an approved workplan and destruction permit issued by PHS-EHD or you may <br /> choose to keep them in place and monitor them and report the findings in a report submitted to PHS-EHD on <br /> an established schedule. <br /> If your choice is to destroy the wells and probes, please submit a short workplan to PHS-EHD along with two <br /> destruction permit applications (one for on-site and another for the off-site well, MW-4)and two separate$60 <br /> checks.
