Fremont, Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS CLASS r MINOR-Notice to Comply) <br /> 605 & 701. (Repeat Violation) Five completely full 55-gallon metal containers had blank or illegible <br /> hazardous waste labels, and one 55-gallon metal container which hazardous label provided only the <br /> generator name and address, and accumulation start date of April 8, 2011, were observed in the back <br /> alley of the facility during this inspection. This violation was previously cited in the 2008 hazardous . <br /> waste inspection which the generator had not returned to compliance to date. Facilities who generate <br /> less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste stored on site at <br /> any time may store waste on site up to 180 days. All hazardous waste containers shall be marked with <br /> the following information: <br /> 1. the words "Hazardous Waste" <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required <br /> information. Since the accumulation start date had been on site longer than 180 days, or are unable to <br /> determine therefore they are considered to be on site longer than 180 days, immediately contact a <br /> licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the <br /> manifest to the EHD by 10/24/2012. <br /> 703 (Repeat Violation) An emergency coordinator and modified contingency plan information is lacking. <br /> There must be at least one emergency coordinator on site or on call to coordinate emergency response <br /> measures, and the following information must be posted by a phone: the name and phone number of the <br /> emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; <br /> and the phone number of the fire department, unless the facility has a direct alarm. Immediately appoint <br /> an emergency coordinator and post the required information by a phone. A form is provided that can be <br /> used for this purpose.
