We are also enclosing an update of recent program changes to <br /> help you complete the renewal and comply with the laws. <br /> COMPLETING YOUR RENEWAL PACKAGE <br /> The enclosed information is based on the notifications you provided in the past. Please <br /> verify the data and, if it does not match your current treatment activities, make appropriate <br /> corrections. Use red ink or otherwise highlight the changes. <br /> After you verify or correct the enclosed information, complete the Certification Page. <br /> Send one copy of the Certification Page, Renewal Form, and attachments (if applicable)to <br /> DTSC's Unified Program Section at the address shown on the Certification Page. Also <br /> mail one complete copy to your Certified Unified Program Agency (CUPA) or other local <br /> agency where there is no CUPA. (For local agency status and address, please see enclosed <br /> list.) Both submissions should be sent by certified mail with return receipt requested. <br /> Do not send any money or your annual notification fee with this renewal form. If the <br /> local agency within your jurisdiction has been certified as a CUPA,then the Board of <br /> Equalization will NOT bill you any State Tiered Permitting fees. You will be billed by <br /> your CUPA. <br /> When you complete this form and mail it as directed,you will satisfy the 1998 annual <br /> notification requirement established by California Code of Regulations, Title 22, Section <br /> 67450.3.
