UST Facility Operating Permit Application is not current in CERS. According to Devon Loomba, in the"UST Facility Operating Permit Application"and the'Business Owner/Operator Identification"sections, the tank operator information,the primary emergency contact information,the secondary emergency contact information and the environmental contact need to be updated with the correct information. Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 106 CCR 2632(d)(2) Failed to have an approved UST response plan. The response plan is not current and not approved by the EHD. The operator stated that Vinay Verma does not work at the facility anymore. He is currently listed as a responsible person and an operator/manager of the tank. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. It was observed that there was a sensor in the Diesel fill sump but the site map lists no sensor in this sump. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a repeat violation, Class ll. 121 CCR 2712(f) Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on 9/28/2016 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 10/28/2016. This information has not been received resulting in a non-compliant status for this facility. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. The violation that was not corrected is the following: The designated operator did not inspect all of the required items. This is a Class II violation.
