WRITTEN MONITORING AND RESPONSE PLAN <br /> UNDERGROUND STORAGE TANK <br /> This monitoring program must be kept at the UST location at all times. The information <br /> on this monitoring program are conditions of the operating permit. The permit holder <br /> must notify Environmental Health Department, (209) 468-3420 within 30 days of any <br /> changes to the monitoring procedures, unless required to obtain approval or a permit <br /> before making the change. Required by CCR 2632 (d) and 2641 (h). <br /> FACILITY F-6 0-D M14/Z T Gc4 So <br /> L <br /> FACILITY ADDRESS 2185 E. FREMONT STREET STOCKTON, CA 95205 <br /> 1. Name(s) and Title(s) of the people responsible for performing the monitoring and/or <br /> maintenance <br /> ADOLPH AND EVANNA EGOROFF <br /> 2. Describe the frequency of performing the monitoring for: <br /> TANKS CONTINUOUS PIPING CONTINUOUS <br /> 3. Describe the following equipment for each underground storage tank. <br /> Name and Model of Monitoring Panel VEEDER-ROOT TLS-350 <br /> Name and Model of Line Leak Detectors FE PETRO MLD-1(GAS) MLD-0(DIESEL) <br /> Name and Model of Annular Sensors V/R #794390-420 Liquid Sensor <br /> Name and Model of Piping Sensors V/R #794380-208 Liquid Sensor <br /> Name and Model of Dispenser Sensors V/R #794380-208 Liquid Sensor <br /> Type of overfill prevention Ball Floats with Veeder-Root overfill alarm 790096-001 and <br /> acknowledgement switch 790095-001 <br /> 5. Do the Turbine Sensors and Dispenser Pan Sensors shut down the turbine? Yes <br /> Does the turbine shut down if a sensor is disconnected? Yes <br /> Does the turbine shut down if the monitoring panel loses power? Yes <br /> 6. How will the alarm history and subsequent maintenance be recorded? <br /> Printout from monitoring system and written log book <br /> 7. Maintenance of the monitoring equipment must be performed at least once every 12 <br /> months. Describe the preventive maintenance schedule and recording for the monitoring <br /> equipment. What month will annual monitoring certification be performed in? November <br /> of each year— Maintenance of monitoring system to be performed by Afforda-Test. <br /> 8. Describe the training necessary for the operation of UST system, including piping, and <br /> the monitoring equipment. Familiarization of the Veeder-Root system console to be done <br /> when monitoring system is started up. <br />
