The white poly 55 gallon drum of used antifreeze near the wiper blade display did not have marked <br /> the hazardous property nor the physical state. The white poly 55 gallon drum of used antifreeze next to <br /> the oil drums did not have marked the hazardous property, the physical state, nor the generator name <br /> and address. All hazardous waste containers shall be marked with the following information: <br /> 1. the words "Hazardous Waste' <br /> 2. name and address of generator <br /> 3. hazardous properties <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> During the inspection the employee Matt Whitson marked the drums of used antifreeze with the required <br /> information. Continue to ensure that all hazardous waste containers are marked with all the required <br /> information. Class II Repeat Violation. Corrected on site. <br /> 32. The hazardous waste tank assessment for the used oil tank is incomplete. The owner or operator <br /> shall obtain a written assessment reviewed and certified by an independent, qualified, professional <br /> engineer, registered in California in accordance with section 66270.11(d) attesting that the system has <br /> sufficient structural integrity, is acceptable for the transferring, storing and treating of hazardous waste, <br /> and that the tanks and containment system are suitably designed to achieve the requirements of this <br /> article. Correct immediately by completing the required hazardous waste tank assessment, keeping a <br /> copy at this facility, and submitting a copy to the EHD. Class II Repeat Violation. <br /> Note: A copy of the hazardous waste tank assessment model and Department of Toxic Substances <br /> Control Tip-5 Tank Integrity Assessments were provided during the inspection. <br /> 49. The personnel training records documenting that site employees were trained within the past twelve <br /> months is not on site. <br /> (a)(1) Facility personnel shall successfully complete a program of classroom instruction or on-the-job <br /> training that teaches them to perform their duties in a way that ensures the facility's compliance with the <br /> requirements of this chapter. The owner or operator shall ensure that this program includes all the <br /> elements described in the document required under subsection (d)(3) of this section. <br /> (1) This program shall be directed by a person trained in hazardous waste management procedures, <br /> and shall include instruction which teaches facility personnel hazardous waste management procedures <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector. /�" Receive0 y: Tige' <br /> EHD 23-02-003 Rev W110110 CONTINUATION FORM <br />
