PHS/EHD has the <br /> following comments for your consideration. I <br /> On October 23, 1992 all monitoring wells with sufficient groundwater were sampled, <br /> MW1, MW4, and MW5. All groundwater samples detected petroleum hydrocarbon <br /> contamination. <br /> PHS/EHD requested in correspondence dated September 15, 1992 that QA/QC data <br /> sheets be submitted with quarterly reports; however, QA/QC data sheets were not <br /> included in the above mentioned quarterly monitoring report. Please be informed that <br /> PHS/EHD has requested QA/QC information from Alpha Analytical Laboratories. <br /> QA/QC data sheets should be included in all future quarterly monitoring reports. <br /> The groundwater contamination plume has not yet been defined: <br /> PHS/EHD requested in correspondence dated September 15, 1992 that a work plan be <br /> submitted by November 16, 1992, in order to continue the groundwater investigation. To <br /> date PHS/EHD has not received the requested work plan. Please submit by February <br /> 22, 1993, a work plan to install a minimum of two additional monitoring wells.
