Proiect Number 1804BK54 April 11,2013 <br /> SITE SAFETY PLAN <br /> Introduction: <br /> This Site Safety Plan (SSP) has been designed to address safety provisions needed <br /> during destruction of groundwater monitoring wells, vapor extraction wells, and soil <br /> vapor survey wells at the Former Busy Bee Cleaners facility, 40 North Main Street, <br /> Lodi, California (Site) (Figure 1). The purpose of this SSP is to provide established <br /> procedures to protect all on-site personnel from direct skin contact, inhalation, or <br /> ingestion of potentially hazardous materials that may be encountered at the site. The <br /> SSP establishes personnel responsibilities, personal protective equipment standards <br /> decontamination procedures, and emergency action plans. <br /> E2C Remediation (E2C) seeks to properly destroy 92 wells. Of those, one (1) is a 6- <br /> inch extraction well, 22 are groundwater monitoring wells, 38 are groundwater air <br /> sparge wells, and 29 are vapor extraction wells. Drilling equipment will be brought to <br /> the site and operated by a subcontractor: <br /> Environmental Control Associates, Inc. <br /> 3011 Twin Palms Drive <br /> Aptos, CA 95003 <br /> C-57 License Number 695970 <br /> The SSP describes means for protecting all on-site personnel from deleterious <br /> contamination or personal injury while conducting on-site activities. Drilling <br /> operations for this project will consist of pressure grouting and/or drilling out <br /> monitoring wells, vapor extraction wells, dual-completion wells, and soil vapor survey <br /> wells. <br /> Site Description <br /> The Site is located in a commercial/industrial area of Lodi (see Figure 2). <br /> Responsibilities of Key Personnel: <br /> All personnel on site will have assigned responsibilities. Mr. Aiguo Xu, P.E., Senior <br /> Associate Engineer, of E2C will serve as Senior Project Manager. Mr. Pete Castro, <br /> C.E.G, Senior Associate Geologist of E2C Remediation will be E2C's on-site <br /> representative and will serve as the Site Safety Officer (SSO). As SSO, Mr. Castro will <br /> present a copy of the SSP to on-site personnel. Personnel will be required to <br /> document their full understanding of the SSP before admission to the site. <br /> Compliance with the SSP will be monitored at all times by the SSO. Appropriate <br /> personnel will conduct a training session to assure that all are aware of safe work <br /> practices. In the training session, personnel will be made aware of hazards at the site <br /> and will utilize Material Safety Data Sheets for information on compounds to be <br /> encountered. <br /> Alternate Project Manager <br /> Mr. Phil Goalwin, P.G. of E2C Remediation, LLC will serve as the alternate project <br /> manager. He will serve as an alternate contact other than the Project Director for <br /> reporting deviations from the SSP and health and safety conditions that may be a risk <br /> to field personnel and as a contact for other project-related decisions. <br /> On-site employees will take reasonable precautions to avoid unforeseen hazards. <br /> After documenting understanding of the SSP, each on-site employee will be <br /> responsible for strict adherence to all points contained herein.
