The permit addendum is <br /> for Phase 3 Groundwater Characterization that includes the installation of fourteen (14) <br /> groundwater monitoring wells and one (1) aquifer test well located in a four-block area in the <br /> vicinity of the Busy Bee site (see attached Figure 2). The Regional Water Quality Control <br /> Board (RWQCB) approved the Workplan for this investigation by letter dated October 20, <br /> 2003 and copies of the approved Workplan and the RWQCB approval letter are provided as <br /> attachments. <br /> The proposed work, as described in the attached Workplan, consists of the installation of <br /> fourteen (14) groundwater monitoring wells and one (1) aquifer test well within a four block <br /> area in the vicinity of the Busy Bee site. The well locations are depicted on attached Figure <br /> 2. Traffic control signage and delineation will be setup as necessary at each location <br /> according to the current CalTrans WATCH manual. Open traffic lanes will be maintained in <br /> both directions during the field operations. As requested by the City, the wells will be <br /> located behind the curb but not within the sidewalk where feasible or alternatively within the <br /> street area near the curb. An encroachment permit application has been submitted to Ms. <br /> Jeannie Matsumoto - Public Works Department, City of Lodi for access to the drilling <br /> locations. <br /> The drilling contractor will be Layne Christensen Company of Fontana, California (C57 <br /> License No. 510011) and their Licensed Contractors Declaration (LCD) has been provided <br /> to your office previously. Underground Service Alert (U.S.A.) will be contacted to field locate <br /> underground utilities prior to commencing the field investigation. Eleven (11) of the borings <br /> (shallow wells) will be advanced using a drill rig equipped with hollow-stem auger equipment <br /> and four (4) of the borings (deep wells and aquifer test well) will be advanced using an air <br /> percussion hammer drill rig.
