Mr. Lim, <br /> I have reviewed the email submittal and have found the following issues: <br /> • Violation #6,The site map has not been uploaded and submitted to CERS;the email copy of the site map does <br /> not show or address the following: <br /> 1. Internal roads. <br /> 2. Emergency response equipment (spill control, absorbent). <br /> 3. Storm drains. <br /> 4. Waste anti-freeze. <br /> S. The map does not accurately depict the topographical information for your site. <br /> 6. Some of the codes/symbols used in the map are not included in the legend. <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br /> access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, <br /> and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br /> label as "NA". <br /> • Violation #11—Closed. <br /> • Violations 2, 3, 9, and 12 will remain open until violation 6 has been successfully addressed. <br /> Be advised,the violation comply by date has passed, consequently all violations cited during the inspection on <br /> November 6, 2019 must be satisfactorily addressed by January 9, 2020; after January 9, 2020, all time spent attempting <br /> to obtain compliance may be billed at current rate of$152 an hour. <br /> As of September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
