(1) Detail of the access road (the first 100 feet from <br /> Waverly Road shall be paved) . <br /> (2) Final grading plan and drainage provision. <br /> (3) Boundaries and quantities of excavation and <br /> direction of excavation. <br /> (4) Stockpile area for topsoil. <br /> b. Loads on Waverly Road shall be restricted to the legal <br /> load limit. <br /> C. An encroachment permit shall be required for work within <br /> the road right-of-way. <br /> d. A video tape of road conditions on Waverly Road from <br /> Shelton Road to Flood Road is required prior to the start <br /> of excavation. The condition of Waverly Road shall be <br /> reviewed with the Department of Public Works prior to the <br /> start of excavation and every five years thereafter. If <br /> it is determined necessary by the Department of Works <br /> during the excavation, the applicant shall construct an <br /> acceleration land and/or a left-turn lane on Waverly Road <br /> to permit safe egress and ingress from the site. Road <br /> construction and design shall be subject to approval by <br /> the Department of Public Works. The applicant shall pay <br /> fees for the plan-check, inspection, and construction <br /> cost. <br /> e. A bond or cash deposit in the amount acceptable to the <br /> Department of Public Works shall be filed with the <br /> Department with a guarantee that if the permittee fails, <br /> refuses, or neglects to repair, under permit, any damages <br /> to Waverly Road which may result from the permittee' s <br /> operations, the Department of Public Works may do the <br /> work necessary to restore Waverly Road equal to, or <br /> better than, its original condition by using the funds <br /> from said bond or cash deposit. <br /> f. A bond or cash "deposit in the amount acceptable to the <br /> Department of Public Works shall be filed with the <br /> Department with a guarantee that if the permittee fails, <br /> refuses, or neglects to reclaim the project site, the <br /> Department of Public Works may do the work of reclamation <br /> by using the funds from said bond or cash deposit. The <br /> progress of reclamation shall be reviewed with the <br /> Department of Public Works every five years and at the <br /> end of excavation. <br /> Conditions of Approval - 3 - (QX-91-6) <br />
