The following is an itemized list of hazardous waste violations that have not been <br /> addressed for Arco. Am pm 83333 as of March 15, 2017. <br /> Open violations from October 30, 2014 inspection <br /> Violation #104-No modified contingency plan. <br /> An emergency coordinator and modified contingency plan information is lacking. There must be at least one <br /> emergency coordinator on site or on call to coordinate emergency response measures, and the following <br /> information must be posted by a phone: the name and phone number of the emergency coordinator; location of fire <br /> extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, <br /> unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required <br /> information by a phone. A form is provided that can be used for this purpose. This is a minor violation. <br /> Violation#110- Failed to keep signed copy of manifests from the designated facility for three years. <br /> 'Copies of uniform manifests for the disposal of hazardous waste for 2013-2014 years were not found on site. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all receipts <br /> used in a consolidated manifesting procedure on site for three years and have them readily available for review. <br /> Immediately locate copies of all manifests and receipts for the last three years, maintain them on site, and submit <br /> copies to the EHD. This is a Class II violation. <br /> Violation #304 - Failed to maintain adequate aisle space for unobstructed response to emergencies. <br /> Three 55-gallon metal containers were observed inside a hazardous waste storage area located north of the <br /> convenience store. Access to one of these containers was obstructed by one of the aforementioned metal <br /> containers storing commingled fuel filters. Adequate aisle space shall be maintained to allow the unobstructed <br /> movement of personnel, fire protection, spill control equipment, and decontamination equipment to all areas of <br /> facility operation in an emergency. Immediately maintain aisle space to allow the unobstructed movement of <br /> personnel, fire protection, spill control equipment, and decontamination equipment to any area of facility operation in <br /> an emergency. This is a minor violation. <br /> Violation #601 -Stored hazardous waste onsite greater than 180 days. <br /> Three 55-gallon metal containers were observed inside the hazardous waste storage area. The first container of <br /> commingled diesel and gasoline filters had an accumulation start date of 12/4/2012. The second container, labeled <br /> "Used oil and gasoline filters", lacked an accumulation start date. Furthermore, due to the restricted access no label <br /> was observed on the third 55-gallon container. Facilities who generate less than 1000 kg of hazardous waste per <br /> month and do not exceed 6000 kg of waste stored on site at any time may store waste on site up to 180 days. <br /> Immediately contact a licensed hazardous waste hauler to dispose of these wastes under manifest and submit a <br /> copy of the manifest to the EHD. This is a Class II violation. <br /> Violation #605 - Failed to completely label containers or tanks of hazardous waste. <br /> At least one of the observed 55-gallon container located inside the hazardous waste storage area north of the store <br /> lacked an appropriate label. All hazardous waste containers shall be marked with the following information: <br /> -the words"Hazardous Waste" <br /> - name and address of generator o <br /> - hazardous properties ;, �. <br /> - physical state <br /> - composition (contents) <br /> -accumulation start date <br /> Immediately label all hazardous waste containers and ensure that all containers are marked with all the required <br /> information. Since it cannot be determined how long these containers has been on site, immediately contact a <br /> licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the <br /> EHD.
