WASTE <br /> LJk - <br /> Pharmacy <br /> Front-End <br /> Notes: <br /> Make sure the accumulation label on the bin has the accumulation start date recorded correctly. Include the store number, <br /> address,phone number,accumulation start date,and last service date. The accumulation start date is the first day an item is <br /> placed in a bin. Leave the accumulation start date blank until accumulation starts. <br /> Ensure the accumulation label on the bin has a last service date recorded. The last service date is the day the hazardous <br /> waste hauler performed the most recent removal of hazardous waste and can be found on the signature line under box 15 <br /> on your most recent manifest in your hazardous waste binder. <br /> When placing waste in the bin,be sure to circle"Solid"or"Liquid"at the top of the label depending on the physical state of the <br /> waste. <br /> Published by Articulate° Storyline 32 4/27/18 <br />
