Appendix A Site Closure Request July 2,2010 <br /> Speedy Foods-Stockton Page 3 of 13 <br /> On January 18, 2005, Ground Zero personnel supervised the installation of vapor extraction well <br /> VW2 by V&W. On February 28, 2005, a Ground Zero technician supervised the destruction of the <br /> groundwater monitoring well MW2 in response to a letter dated February 15, 2005 sent by EHD <br /> requesting destruction of the well to facilitate the Highway 99 Interchange Project being directed by <br /> CalTrans. A Solleco®thennal oxidizer soil vapor treatment system(VTS) was placed in operation on <br /> August 8, 2005 connected by sub-grade piping to two vapor extraction wells (VW1 and VW2), a dual- <br /> phase extraction well (EW1), and two monitoring wells (MW3 and MW4). Details are discussed in <br /> Ground Zero's Quarterly Monitoring Report January through March 2005 dated May 2, 2005 and <br /> Quarterly Monitoring Report July through September 2005 dated November 22, 2005. <br /> Ground Zero submitted the Feasibility Study - Enhancement of Existing Remediation Activities <br /> dated September 25,2007 which was approved by the EHD in a letter dated October 22, 2007. <br /> Ground Zero personnel supervised the installation of three groundwater monitoring wells between <br /> October 9 and 25, 2007 by V&W. V&W installed monitoring wells MW 19D (D Zone aquifer), <br /> MW18C (C Zone aquifer) and MW17B (B Zone aquifer). Dewey Data Inc. of Stockton, California <br /> performed an a-log of MW19D. On November 7, 2007, Del-Tech developed the new monitoring <br /> wells. On November 2, 2007, Mr. Vasquez supervised the advancement of two soil confirmation <br /> borings (CBI and CB2) by V&W. The confirmation borings were advanced in the location of the <br /> former USTs. Details are discussed in Ground Zero's Monitoring Well Installation and Soil <br /> Investigation Report-Fall 2007, November 28,2007. <br /> On December 4, 2007, Ground Zero personnel supervised the installation of groundwater extraction <br /> well EW3 by V&W. The well was surveyed by Morrow Surveying on December 1, 2007. On January <br /> 17, 2008, Del-Tech developed well EW3. Details are discussed in Ground Zero's Quarterly <br /> Groundwater Monitoring and Treatment System Operational Report October through December 2007 <br /> dated February 6, 2008 and Quarterly Groundwater Monitoring, Groundwater Treatment System <br /> Operation and Effectiveness, Soil Vapor Extraction System Rebound Test, aid Additional Site Work <br /> Report-January through March 2008 dated April 7, 2008. <br /> Site closure was requested in Ground Zero's Site Closure Request and Semi-Annual Groundwater <br /> Monitoring Report- Winter/Spring 2010, dated March 24, 2010. In a letter dated June 1, 2010, EHD <br /> directed the submittal of a work plan to begin destroying the wells no longer necessary for the site <br /> investigation. Ground Zero submitted the Groundwater Monitoring Well Destruction Workplan dated <br /> June 9, 2010. The June 1, 2010 letter also request additional information be submitted following the <br /> requirements of the Tri-Regional Guidelines Appendix A. This document provides the additional <br /> information required to continue the review for low-risk closure. <br />
