Eyewashes. <br />*10 <br />The owner or operator shall provide to personnel working in the <br />hazardous waste area of the facility adequate numbers of eyewashes. <br />The eyewashes shall be in workable condition and available for <br />immediate use. <br />14. First-aid kit. <br />The owner or operator shall provide to personnel working in the <br />hazardous waste area of the facility adequate numbers of industrial - <br />type first-aid kits. The kits shall be maintained and available for <br />immediate use. <br />15. Protective clothing. <br />The owner or operator shall provide to personnel working in the <br />hazardous waste area of the facility adequate numbers of the fol- <br />lowing National Institute of Occupational Safety and Health (NIOSH) <br />approved equipment if appropriate: <br />(a) Protective head gear and face masks. <br />(b) Chemically resistant apparel and gloves. <br />(c) Self-contained breathing apparatus and respirators with the <br />approved cartridges. <br />16. Recordkeeping and reporting. <br />The owner or operator shall submit to the California State Department <br />of Health Services by the last day of each month: <br />(a) A record of all hazardous waste disposed of during the previous <br />month, showing the identity, source, chemical composition, weight <br />or volume, physical state, container type, hazardous properties <br />and method used to dispose of each waste. <br />(b) A report showing the amount of fees due and payable to the Depart- <br />ment for the previous month. <br />(c) Payment for fees due and payable to the Department in conformance <br />with Article 8, Chapter 30, Division 4, Title 22, California Admin- <br />istrative Code. <br />The required information ((a) and (b)) shall be reported on a form <br />approved by the California State Department of Health Services. <br />
