This work was completed without EHD <br /> permit between January 2017 to January 2018. The permit has been approved with the following conditions: <br /> 1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br /> applicable permits and requirements. <br /> 2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br /> commencing work. All new addenda supersede the previous ones. <br /> 3. Contact Zuna Barker at(209)953-7311 to arrange the following appointments(a 48-hour notice is <br /> required for inspection appointments). Please be aware that inspections for emergency work must be <br /> scheduled within five days of permit approval. <br /> • Sensor functionality: Functionality of the monitoring system must be verified to ensure that <br /> monitoring has been restored to meet the requirements of Title 23 California Code of Regulations <br /> and the Health&Safety Code. If it is determined that a cold start is necessary, a copy of the <br /> alarm history shall be printed prior to performing the"cold start"of the monitoring system and <br /> made available at the time of EHD inspection. <br /> • Visual inspection of UDC 7/8 for brine leaks. UDC 5/6 brine sensor communication, sensor <br /> functionality verification,and alarm history verification for brine leaks. <br /> • Spill bucket test:Verify minimum 5-gallon capacity and pass a one hour hydrostatic test(or <br /> equivalent). Proper drainage must also be verified. <br /> Please be aware that the facility cannot receive fuel in the Diesel and 91 product tank prior to <br /> the EHD inspection and approval. <br /> 4. If concrete will be broken near the Diesel and 91 product tank, a secondary containment test must be <br /> performed on the Diesel and 91 product tank once concrete is poured in accordance with the <br /> manufacturer's guidelines. <br /> 1868 E.
