Service was not performed by LC Services for the S6 and S7 vacuum alarms or the UDC
7/8. 1 will reach out to Boyett to see if we can locate a work order for the work done on UDC 7/8 by another contractor.
(Monica or Rick can you please provide?)
Per our conversation, functionality of all of the above will be checked and confirmed during the scheduled Monitor
Certification Test with Service Station Systems on 08/29/18.
Please let me know if there is anything else you need from me.
Thank you,
Hope Vargas
Office Manager
L.C. Now I would need the scope of work for the SR0079109 based on our conversations from yesterday. Once I
have that I will process the approval letter and correct some of those violations. I remember you stating that there is no
work order for repairs conducted on UDC 7/8. While the inspector was on site on 9/7/17 he noted that very little
evidence of brine was presence in UDC 7/8. We do not have records that anything was done to correct this problem. We
received a letter from Rick Jewell (see attached) stating that the liquid in UDC 7/8 could be multiple of liquids, water,
condensation etc. However in the inspection report is noted that there was a lack of brine in UDC 7/8 and not liquid
inside UDC 7/8. SJCEHD would like to documentation of what was done to correct why there was a lack of brine in UDC
7/8. Please provide that documentation so that we can close violation 305.
1
