San Joaquin County
Environmental Health Department
1868 East Hazelton Avenue, Stockton, California 95205-6232
Telephone: (209) 468-3420 Fax: (209) 468-3433 Web: www.sigov.org/ehd
RCRA Large Quantity Hazardous Waste Generator Inspection Report
Facility Name:
Facility Address:
Date:
RITE AID #5996
1050 N WILSON WAY, STOCKTON
September 27, 2018
SUMMARY OF VIOLATIONS
(CLASS I, CLASS II, or MINOR - Notice to Comply)
Item #
Remarks
109
CCR 66265.52 Contingency plan is incomplete.
The facility's contingency plan is incomplete. The address for the second alternate emergency coordinator, Jeff
Drake, was missing from the contingency plan (see photo IMG 2084). The contingency plan must include:
1. Description of actions facility personnel will take in response to fires, explosions, or any sudden or
non -sudden release of hazardous waste to air, soil or surface water at the facility;
2. Description of arrangements made with local police departments, fire departments, hospitals, contractors and
State and local emergency response teams to coordinate emergency services;
3. List of names, addresses, and phone numbers (office/home/cell) of all persons qualified to act as emergency
coordinator. Where more than one person is listed, one shall be named as the primary emergency coordinator and
all others in order in which they will assume responsibilities;
4. List all emergency equipment, which includes location, physical description, and brief outline of its capabilities;
5. Evacuation routes for facility personnel where there is a possibility of evacuation;
6. Current telephone number for State Office of Emergency Services.
If the facility has a Spill Prevention, Control and Countermeasures plan, or some other emergency or contingency
plan, the owner or operator need only amend that plan to incorporate hazardous waste management provisions that
are sufficient to comply with the requirements of this chapter. Chris Knight wrote Jeff Drake's address into the
contingency plan at the time of inspection (IMG 2086).
This was corrected on site.
This is a Class II violation.
113
CCR 66262.23(a) Failed to complete, sign or date manifest; or obtain dated signature of transporter.
Manifest 017478482JJK (7/2/2018) lacked waste codes (see photo IMG 2069). The generator and waste sections
of the uniform manifest shall be completed and signed and dated by the generator. The transporter shall also sign
and date the manifest.
Ensure that all manifests are properly completed for any transport of a hazardous waste for off-site transfer,
treatment, storage or disposal.
This is a Class II violation.
FA0001105 PRO536074 SCO01 09/27/2018
EHD 22-03 Rev. 09/22/16 Page 5 of 9 RCRA Large Quantity Hazardous Waste Generator OIR
