Main St. <br /> Stockton, CA., 95202 <br /> As the responsible party,you were notified on November 6, 1998 that the illegally abandoned underground waste oil <br /> storage tank(UST)was to be brought into compliance by December 6, 1998 or a closure plan and all fees should be <br /> submitted to San Joaquin County,Public Health Services,Environmental Health Division(PHS-EHD),by December 6, <br /> 1998. Neither requirement has been complied with. <br /> On December 11, 1998 PHS-EHD sent you another notice and all of the forms to be completed and submitted in order <br /> t to bring your site into compliance. The deadline was reset for December 21, 1998.
